Neurophysiological Responses for Better Understanding the Antalgic Mechanisms of Spinal Cord Stimulation
نویسنده
چکیده
Spinal cord stimulation (SCS) is a physical invasive therapy largely used to treat patients with neuropathic pain refractory to pharmacologic treatments. Its mechanisms of action are not completely understood and, so far, patients continue to be enrolled with a trial and error approach. SCS is based on the electrical stimulation of dorsal columns, which evokes a typical electrical paresthesia, similar to that experienced by any person when an electrical stimulation is applied over the skin. This electrical sensation is not physiological because it is the result of an ectopic, direct activation of nerve fibers with a by-pass of the receptor’s activation. The electrical stimulation of dorsal columns induces action potentials in the lemniscal pathway, a “system” made by fibers with large diameter and high conduction velocity. As any other electrical stimulation of nerve fibers, SCS creates potentials which travel in opposite direction: cranially and caudally. Afferent, orthodromic propagation of action potentials during SCS is mandatory for any supposed antalgic mechanism occurring in brainstem or brain, while antidromic propagation is considered the basis for the antalgic, segmental effect of SCS. Studies on neurophysiological effects of SCS are rare and further investigations are warranted in order to achieve a better understanding of the antalgic effects of SCS, and consequently improve therapeutic targeting and patient selection. *Corresponding author: Michelangelo Buonocore, Unit of Clinical Neurophysiology, Fondazione Maugeri Via Maugeri 10, 27100, Pavia, Italy, Tel: +39 0382 592 392, Fax: +39 0382 592 020; E-mail: [email protected] Received March 31, 2013; Accepted June 25, 2013; Published June 27, 2013 Citation: Buonocore M (2013) Neurophysiological Responses for Better Understanding the Antalgic Mechanisms of Spinal Cord Stimulation. متن کاملCellular and Molecular Mechanisms Involved in Neuroinflammation after Acute Traumatic Spinal Cord Injury
Introduction: Spinal cord injury (SCI) following traumatic events is associated with the limited therapeutic options and sever complications, which can be partly due to inflammatory response. Therefore, this study aims to explore the role of inflammation in spinal cord injury. The findings showed that the pathological conditions of nervous system lead to activation of microglia, astrocyte, neut...
متن کاملComparison of Agonist vs. Antagonist Stimulation on Triceps Surae Spasticity in Spinal Cord
Objectives: One of the most common and disabling complications that affects individuals with spinal cord injury is spasticity. The purpose of this study is to compare the effect of agonist and antagonist electrical stimulations on triceps surae muscle spasticity in patients with spinal cord injury. Methods: A total of 30 subjects with spinal cord injury were considered for the study. They were...
متن کاملTrans‐spinal direct current stimulation modifies spinal cord excitability through synaptic and axonal mechanisms
The spinal cord is extremely complex. Therefore, trans-spinal direct current stimulation (tsDCS) is expected to produce a multitude of neurophysiological changes. Here, we asked how tsDCS differentially affects synaptic and nonsynaptic transmission. We investigated the effects of tsDCS on synaptically mediated responses by stimulating the medullary longitudinal fascicle and recording responses ...
متن کاملCommon neural structures activated by epidural and transcutaneous lumbar spinal cord stimulation: Elicitation of posterior root-muscle reflexes
Epidural electrical stimulation of the lumbar spinal cord is currently regaining momentum as a neuromodulation intervention in spinal cord injury (SCI) to modify dysregulated sensorimotor functions and augment residual motor capacity. There is ample evidence that it engages spinal circuits through the electrical stimulation of large-to-medium diameter afferent fibers within lumbar and upper sac...
